Nurse Practitioner Job Summary: We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health in Houserville, Pennsylvania. As a Nurse Practitioner with our organization, you will have the opportunity to deliver high-quality preventative care to residents in skilled nursing facilities, while enjoying a hybrid work arrangement that combines on-site visits with remote work from home. Our team is dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care. Responsibilities: - Provide preventative care to residents in skilled nursing facilities, including conducting on-site visits and remote monitoring of vital signs - Collaborate with healthcare teams to develop and implement individualized care plans, while ensuring continuity of care and coordination with other healthcare providers - Conduct documentation and charting in a timely and accurate manner, with the support of a dedicated scribe team - Participate in multidisciplinary team meetings and clinical decision-making, utilizing your expertise in Geriatrics and Internal Medicine - Build strong relationships with facility staff, patients, and families, while maintaining a patient-centered approach - Stay current with industry developments and best practices through ongoing education and training Requirements: - Active NP or PA license in the state of practice - 1+ year of experience in Geriatric care and documentation review - Excellent communication, collaboration, and teamwork skills - Ability to work independently and as part of a team - Strong problem-solving and critical thinking skills Additional Information: This is a full-time position offering a competitive salary ranging from $120,000 to $125,000 annually.
