Pharmacist, Ambulatory Clinical Services

Pharmacists are integral members of the health care team at UC Davis Health. They share responsibility for comprehensive medication management, distribution of medications to inpatients and ambulatory patients, oversight and development of treatment guidelines and formulary management, stewardship of high-cost drug therapy, teaching for pharmacy students, residents and staff, clinical research, participation in Department and Hospital Committees, task forces, strategic planning and key initiatives related to pharmaceutical care. This position works in multiple ambulatory clinical specialties. The pharmacist's responsibility is to monitor drug therapy to achieve desired outcomes and to avoid drug toxicity and the dosing of medication based on patient demographics, available treatment guidelines and response. The pharmacist works as an integral part of the patient care team and assists physicians and nurses in providing drug therapy.

Minimum Qualifications:

• California Pharmacist License eligible and possess a valid license at the time of hire
• American Heart Association (AHA) or American Red Cross Basic Life Support (BLS), or UC Davis Health CPR
• Doctor of Pharmacy Degree (PharmD), or equivalent training
• Completion of a Pharmacy Practice Post Graduate Year 1 (PGY1) residency, or 3 years of Pharmacist experience
• Thorough understanding and competency with aseptic technique and safe handling of hazardous meds
• Computer skills that facilitate clinical patient data medication order entry and other basic clinical software programs
• Demonstrated ability to effectively communicate with and educate patients about their medication therapy
• Written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English
• Knowledge of current ambulatory care guidelines (i.e., NCEP, JNC, ADA)
• Demonstrated ability to identify medication-related problems in patients, communicate them to a patient's physician and offer recommendations
• Complete working knowledge of, and skill to apply, practices, principles and techniques of patient care related to area of expertise
• Decision-making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached

Preferred Qualifications:

• Pharmacist Board Certification [e.g., Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Ambulatory Care Pharmacist (BCACP), or Certified Specialty Pharmacist (CSP)]
• Experience working with and directing residents, students, and health care professionals
• Completion of a Pharmacy Practice Post Graduate Year 2 (PGY2) residency, or 5 years of Pharmacist experience
• Experience with navigating third party reimbursement programs

We also have the largest pharmacy residency program in the country with 46 total residents in 13 different programs. The Department of Pharmacy offers comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps, and teaching) throughout the health system complex 24 hours a day, 7 days a week. The pharmacy services encompass inpatient pharmacy services as well as an expansive footprint in the ambulatory care space where we have clinical pharmacists embedded within our clinics and specialty care locations. Our operational teams support the health system enterprise with three inpatient pharmacies, five outpatient pharmacies, two infusion pharmacies, a home infusion service line, and the full complement of pharmacy purchasing, stores, and distribution.
