Medical Social Worker

FT, 80 hrs/pp, 1.0 FTE; varies as scheduled.Job Summary:Responsible for Care Coordination to the patients of the hospital and outpatient services at Prosser Memorial Health. Specific duties include crisis intervention, emotional support, establishing linkages with community resources and discharge planning. Provides case management for the Emergency Department Information Exchange (EDIE) and referrals to the Consistent Care program. Provides support to the Director of Care Coordination r in maintaining the operations, policies and procedures for the Social Service Program.Education and/or Experience Requirements:Master’s or bachelor’s degree in social work, or other Health Related Degree.Three years in social service or health care setting required including experience with counseling and discharge planning.Ability to recognize age specific needs and behaviors. Ability to communicate both verbally and through writtenreports with strong documentation skills. Good interpersonal and time management skills.Licensure & Certifications Requirements:Registered to practice in the State of WA as a Licensed Independent Clinical Social Worker, Social Worker or AgencyAffiliated Counselor or achieve such certification within sixty (60) days of employment.Pay Range: $29.69-$48.06 #J-18808-Ljbffr
