Position Summary: LPN – Licensed Practical Nurse

As an LPN at Charter Senior Living , you are a cornerstone of compassionate care in our Assisted Living and Memory Care programs. You’ll provide hands-on nursing support, medication administration, care plan implementation, and leadership to ensure each resident enjoys a safe, fulfilling, and meaningful life.

You’ll also play a key role in promoting teamwork, mentoring caregivers, and maintaining strong communication with residents, families, and staff.

Key Responsibilities

• Administer medications and treatments in accordance with state regulations and company standards

• Conduct resident health assessments and contribute to individualized care plans

• Monitor resident condition changes and communicate promptly with leadership and families

• Supervise and support caregivers and CNAs to ensure quality care delivery

• Document all nursing services, assessments, and interventions accurately

• Uphold infection control and safety protocols to protect residents and staff

• Engage residents with patience, respect, and kindness, enhancing their overall quality of life

Qualifications

✅ License: Current LPN license in the State of Kentucky (required)
✅ Experience: Prior nursing experience in senior living, long-term care, or memory care preferred
✅ Skills: Strong clinical, communication, and leadership skills
✅ Schedule: Ability to work 12-hour shifts (Day or Night) with reliability and flexibility
✅ Attributes: Compassionate, dependable, and team-oriented
