RN Case Manager, Hospice Care with Impact

Grace Hospice in Cincinnati, OH is seeking an RN Case Manager to provide compassionate end-of-life care in patients’ homes and community settings, delivering intermittent skilled nursing as needed. You will supervise hospice aides, coordinate care plans, support families through highly sensitive experiences, participate in on-call duties, and collaborate with the triage team to admit eligible patients.
