Compassionate Hospice RN Case Manager

SouthernCare is seeking aHospice RN / RN Case Manager to lead patient assessments, develop and update plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains. You will educate and support patients and families, ensuring compassionate, comprehensive end-of-life care. The role requires a Registered Nurse with 1+ year of RN experience; hospice or palliative care experience is preferred.
