Position Overview The Nurse Practitioner (NP) will provide comprehensive medical care services to youth within a juvenile correctional setting in accordance with regulatory standards and established clinical protocols. The NP will be responsible for assessing patient needs, diagnosing illnesses, ordering and interpreting diagnostic tests, developing treatment plans, and collaborating with licensed physicians to ensure high-quality healthcare delivery. The ideal candidate will possess strong clinical assessment skills, experience managing diverse patient populations, and the ability to work effectively within a multidisciplinary correctional healthcare environment.

Key Responsibilities • Assess, evaluate, and manage the healthcare needs of youth residing within the facility. • Obtain comprehensive health histories and perform physical examinations. • Order, perform, and interpret diagnostic and laboratory tests to support clinical decision-making. • Diagnose acute and chronic illnesses, injuries, and medical conditions. • Develop, implement, and monitor individualized treatment plans. • Prescribe medications, therapies, and other interventions in accordance with New York State regulations and established practice agreements. • Collaborate with licensed physicians under written practice agreements and practice protocols. • Perform therapeutic and corrective measures within the scope of Nurse Practitioner practice. • Monitor patient progress and modify treatment plans as clinically appropriate. • Maintain accurate, complete, and timely medical documentation and patient records. • Participate in interdisciplinary treatment planning and healthcare team meetings. • Provide health education and counseling to patients regarding disease prevention, treatment compliance, and overall wellness. • Provide after-hours and weekend telephone consultation for urgent health situations as required.
• Perform additional clinical duties necessary to support patient care and facility healthcare operations.

Work Schedule & Conditions • Schedule: Monday - Friday • Shift Hours: 8:00 AM - 4:30 PM EST • Hours: 40 hours per week • Includes a 30-minute unpaid meal break • Overtime: Not required • Travel: Not required • Holidays: Required to work New York State holidays as scheduled

Facility Information • Industry Residential Center is located approximately 16 miles south of Rochester, New York. • Facility includes both secure and limited secure settings. • Serves male juvenile offenders placed by Family Court under the custody of the Office of Children and Family Services (OCFS). • Youth may remain in OCFS custody until age 21, depending on sentencing requirements. • Free on-site parking available. • Dress code: Business casual attire or scrubs. • Supervision staff will be present when healthcare services are provided to youth. • Currently, there are no vaccination requirements.

Qualifications & Requirements • Current licensure as a Family Health Nurse Practitioner (FNP) in the State of New York required • Must maintain an active New York State license and registration throughout the assignment • Candidate must have completed a minimum of 3,500 hours of physician collaboration as required • Resume must clearly document completion of the required 3,500 collaboration hours • Strong clinical assessment, diagnostic, treatment planning, and patient management skills • Experience in correctional healthcare, pediatrics, adolescent medicine, family practice, behavioral health, or residential healthcare settings preferred • Excellent communication, documentation, and interdisciplinary collaboration skills • Commitment to patient-centered care, professional ethics, and regulatory compliance

Background Check Requirements • Must successfully complete all facility-required background checks and security clearances prior to assignment

Vaccination & Medical Requirements • Currently, there are no vaccination requirements

Required Training • Facility-specific orientation and training as required prior to assignment
