Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the lifespan.
The LPN must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
Direct and indirect skilled nursing care will be provided for assigned residents.
The LPN follows defined policies and procedures under the supervision of a Registered Nurse.
A collaborative relationship must be maintained with other nursing staff, physicians, and ancillary staff.
The LPN should maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
The ability to provide care to geriatric residents is essential.
Job duties include collecting data relevant to residents' physical and psychosocial conditions on an ongoing basis.
Findings must be communicated to the Registered Nurse and other members of the Interdisciplinary Team.
The LPN contributes to the creation of a developmentally appropriate and culturally relevant plan of resident care.
This plan should reflect nursing diagnoses and interventions to achieve desired resident outcomes.
Involvement of the resident and family, along with the healthcare team, in this process is important.
Revisions to the plan of care should be recommended after evaluation of resident outcomes.
The LPN indicates the need for referrals to ensure all aspects of the resident's care are addressed.
Collaboration with the resident, family, and members of the Interdisciplinary Team is required to assure customer satisfaction.
The LPN must maintain current knowledge and competency in Nursing Practice.
Participation in the mentoring and orientation of staff is expected.
Factors related to safety, cost, and impact should be considered in planning the delivery of care.
Under the direction of the Registered Nurse, the LPN provides leadership in the professional practice setting.
Demonstration of knowledge and competency is crucial.
Occupational exposure to bloodborne pathogens is anticipated.
Required tasks may involve potential contact with blood, body fluids, or tissues.
