Primary Care Physician

Primary Care Physician (PCP) We’re changing lives every day for both our patients and our team members. We’re rapidly expanding and need primary care physicians who are innovative, entrepreneurial, and dedicated to providing high-quality, affordable VIP care for seniors. Essential Job Duties / Responsibilities The PCP independently provides care for patients with acute and chronic illnesses in the older adult population. The PCP takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved in the detailed care of the patient. The PCP engages with the hospitalist for hospitalizations, regardless of the hospitalist’s affiliation. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination and documentation for complex chronic health needs. The PCP leads a care team that includes a medical assistant (care promoter), care facilitator, and care coordinator for office-based patients. For patients who cannot come to the office (hospital, SNF, LTC, homebound), the PCP engages with a transitional care team and other resources. The PCP has an active role in center management, covering for other providers, recruiting patients, and recruiting additional providers as needed. The PCP collaborates with or functions under an advanced practice practitioner, remaining accountable for the APP’s actions. Performs other duties as assigned and modified at the manager’s discretion. Knowledge, Skills & Abilities Availability and accessibility: open to walk-in visits and responsive to calls and messages. Service orientation: provides care that a family member would expect. Evidence-based medicine: stays updated on medical, nutritional, and lifestyle-based interventions. Leadership: continuously develops and improves leadership skills for patients, the team, the center, and the company. Quality focus: works with care teams to achieve high-quality outcomes and improve practice and operations. Influence: competently and compassionately influences patients, teams, and oneself. Self-care: maintains physical, mental, social, and spiritual well-being. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other relevant software. Willingness to travel locally, nationally for initial training (2 weeks), and occasionally regionally or nationally for recruiting or training purposes. Fluency in English, verbal and written. Fluency in other languages may be required in some centers. Use of independent judgment in all aspects of patient care and practice management. Pay Range $221,141 - $315,915 Salary (base annual full-time salary). Final compensation will depend on experience, education, geographic location, and other factors.
