Job Title: RN Hospice Case Manager Profession: RN Specialty: Hospice Duration: 8 weeks, with the option to extend Shift: AM Hours per Shift: 8 hours Experience: Prior hospice nursing experience Strong case management and interdisciplinary team collaboration skills Ability to manage a hospice caseload independently while providing compassionate, patient-centered care License: Active and clear RN license Certifications: Associates or Bachelor's degree Must-Have: Experience with Homecare Homebase (HCHB) Description: RN Hospice Case Manager needed for an 8-week contract with the option to extend. Schedule: Monday to Friday, 8 AM to 4:30 PM, 40 hours per week. No on-call duties required. Candidates must hold an active and clear RN license. Educational qualifications must include either an Associates or Bachelor's degree. Prior experience in hospice nursing is essential. Familiarity with Homecare Homebase (HCHB) is required. Candidates should possess strong case management and interdisciplinary team collaboration skills. This role involves managing a hospice caseload independently while delivering compassionate, patient-centered care.
