Collaborates closely with all disciplines to ensure patient needs are met in accordance with plan of care. Maintains clinical records in accordance with New York State and Federal regulations, and Home Care policy. Supports organization’s mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification. Job Duties / Responsibilities Provides Home Care nursing services in accordance with the plan of treatment signed by the physician, and makes the initial evaluation visit to the patient as needed. Collaborates with team members, physicians and patients/caregivers to re‑evaluate the nursing needs of the patient; initiates, develops, implements and revises the patient’s plan of care; assesses ongoing needs; addresses plan issues or documents rationale; completes MD orders and 60‑day summary per regulatory guidelines. Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate; makes referrals to other disciplines when indicated by patient needs and physician orders, or documents rationale. Observes signs and symptoms and reports to the physician or other appropriate health professionals as needed, or upon changes in patient’s condition. Teaches, supervises and counsels patient and family regarding home nursing procedures and other care needs; utilizes agency educational material. Coordinates total plan of care for all RN’s caseload, maintains continuity by collaborating with other health professionals; attends patient care conferences when needed; initiates conferences for complex patients as required. Maintains collaborative relationship with Supervisors for care coordination in line with Home Care policy. Supervises and teaches the home health aide every two weeks if required for a field visit; assesses need for unskilled care and revises plan accordingly; communicates plan changes to the MD as needed. Develops, prepares and maintains individualized patient care progress records accurately and timely; performs all nursing visits per agency policies and procedures; documents accurately and submits on time. Ensures compliance with all agency policies regarding service delivery, documentation and care coordination; during supervisory visits demonstrates full compliance and knowledge of policies including patient education, infection control, hazardous waste management and age‑specific behaviors. Demonstrates competency in all skills required, including expertise in pediatrics. Participates in an on‑call rotation. Performs ongoing record review for quality and regulatory compliance. Ensures signed MD orders are generated per policies no less than every 60 days and as needed. Meets established productivity standards. Participates in the agency’s quality improvement program. Participates in appropriate community activities as requested. Performs other duties as required. Requirements Associate’s degree in Nursing; B.S.N. preferred. Valid Registered Nursing license to practice in New York State. Minimum of one year of nursing experience in pediatrics. Home care experience preferred.
