Registered Nurse - Hospice

Job Description

Job Summary:
The Hospice RN - Case Manager delivers comprehensive nursing, care coordination, and case management services to hospice patients in their homes. This role involves assessing patient needs, developing care plans, and coordinating interdisciplinary services to ensure quality, compassionate care and positive clinical outcomes. The position may transition from a Visiting RN to a full Case Manager.

Responsibilities:

• Assess patients' physical, emotional, and psychosocial health status to develop individualized care plans.
• Coordinate and oversee care provided by Home Health Aides and CNAs to ensure adherence to care plans.
• Communicate effectively with interdisciplinary teams, community agencies, and family members to maintain continuity of care.
• Document patient records accurately, complying with agency standards, OASIS requirements, and reimbursement regulations.
• Participate in on-call rotations, visiting patients as needed based on assessment and care priorities.
• Contribute to quality improvement initiatives and attend ongoing training and in-service sessions.

Qualifications:

• Hold a current RN license in Montana; BSN preferred.
• Possess a valid driver's license, reliable transportation, and appropriate auto insurance.
• Have at least 2 years of recent clinical experience in home care, hospice, or acute care settings.
• Demonstrate proficiency in clinical skills such as wound care, venipuncture, catheterization, and parenteral therapy.
• Experience with HCHB is highly preferred.
