Be a part of our team of healthcare heroes and discover why Catholic Health was named Long Island's Top Workplace.Job DetailsOur Nurse Transition Coach will:Collaborate and coordinate with multidisciplinary teams to facilitate a safe quality experience for patients through the continuum of careAct as a liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum.Communicate/collaborates daily with physicians, unit and support team (MSW, CDI, PT, etc.) to progress each patient's care towards discharge and resolve barriers to dischargeEscalate complex patient care coordination needs to appropriate support team memberServe as an expert resource in patient flow, status and progression and provides ongoing education to staff, physicians, patients, and their significant othersEnsure designated software/EPIC reflects accurately the patient status and is updated in real time as neededReviews Target LOS values and working DRG daily and at points of patient condition changesFacilitate family meetings and plan of care discussions related to supportive care, healthcare proxy and guardianshipWork collaboratively with care team to facilitate financial assistance, entitlements and medication assistance as requested. Provide community resource information as requested and facilitates or completes referrals as directedPerform the Care Management functions and documentation as per orientation and competency documentsMeet deadlines and exceeds metrics identified for the Facility/ Unit through implementation of facility policies and Standard Operating Procedures (SOPs)Participate or lead meetings as requiredQualificationsWhat you'll bring:3+ years in-patient acute care clinical RN experience requiredExperience in Care Management or Quality preferred2+ years progressive nursing leadership experience preferredEducation: Graduate of an accredited School of Nursing requiredBachelors of Science in Nursing (BSN) degree preferredLicensure: New York Registered Nurse (RN) License & RegistrationCertification: Related specialty certification preferred; Case Management Certification preferred; PRI Certification preferredSkills: Beginner proficiency of Outlook, Word and Excel is required.
