Your Role

Were seeking a compassionate, experienced Home Health Physical Therapist Assistant (PTA) to join our high-performing team. The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. Youll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy.

What Youll Do

Provide skilled physical therapy treatments in patients homes under the direction of a licensed Physical Therapist
Carry out individualized treatment plans focused on mobility, strength, balance, and functional independence
Assist patients with therapeutic exercises, gait training, transfers, and mobility training
Educate patients and caregivers on safe mobility techniques, home exercise programs, and fall prevention strategies
Observe, document, and report patient responses and progress to the supervising Physical Therapist
Communicate effectively with physicians, nurses, and other members of the interdisciplinary care team
Maintain accurate, timely, and compliant clinical documentation
Ensure compliance with all Medicare, CMS, and home health regulations

What Were Looking For

Current Florida Physical Therapist Assistant (PTA) license (required)
Previous home health experience is strongly preferred but not required
Excellent communication, time management, and documentation skills
A compassionate, dependable, and professional attitude
