Hospice RN Case Manager — Flexible Schedule & On-Call
Infinity Hospice Care LLC
Infinity Hospice Care LLC is seeking a skilled RN Case Manager to perform admissions and coordinate nursing care for patients at home. You will work with the Interdisciplinary Group/Team, patients and families to assess needs, develop plans of care, and ensure timely documentation. The role requires current Texas RN license, hospice experience, ability to travel in the Dallas area, and participation in QAPI and in-service programs.
