Hospice Registered Nurse (RN) Case Manager

Multiple Openings – Greater Chicago Metropolitan Area, Illinois

Annual Salary: $80,000 + Additional On-Call Compensation
Employment Type: Full-Time | 40 Hours Per Week
Schedule: Monday–Friday, Generally 8:00 AM–5:00 PM
Work Setting: Field-Based / Home Hospice Care

Position Overview

Mathis Consultants is seeking compassionate and experienced Hospice Registered Nurse (RN) Case Managers for multiple full-time opportunities with an established home health and hospice organization throughout the greater Chicago metropolitan area.

This position is ideal for an experienced hospice nurse who is passionate about providing exceptional end-of-life care, coordinating interdisciplinary healthcare services, and supporting patients and their families during challenging transitions.

The selected candidates will manage assigned hospice patient caseloads, conduct clinical assessments, develop individualized care plans, and collaborate with healthcare professionals to ensure patients receive compassionate, high-quality care in the comfort of their homes.

Key Responsibilities

• Manage and coordinate individualized hospice care for assigned patients.
• Conduct comprehensive nursing assessments and develop, implement, and update patient care plans.
• Monitor patient conditions, assess symptoms, and provide skilled nursing interventions focused on comfort and quality of life.
• Coordinate patient care with physicians, hospice aides, social workers, chaplains, and other interdisciplinary team members.
• Educate patients and families regarding disease progression, symptom management, medication administration, and end-of-life care.
• Provide emotional support, education, and guidance to patients and their caregivers.
• Coordinate patient visits, clinical services, and transitions of care to ensure continuity.
• Provide clinical leadership, guidance, and support to nursing staff and other care team members.
• Participate in interdisciplinary team meetings, case conferences, and care planning discussions.
• Maintain accurate and timely clinical documentation in accordance with applicable regulations and organizational policies.
• Respond to patient needs during assigned on-call rotations.
• Travel throughout the assigned service territory to deliver direct patient care.

Required Qualifications

• High school diploma or GED.
• Active, valid Registered Nurse (RN) license in Illinois.
• Minimum of 1 year of hospice nursing experience.
• Minimum of 1 year of management, supervisory, or clinical leadership experience.
• Minimum of 1 year of home health or home-based healthcare experience.
• Current American Heart Association Basic Life Support (AHA BLS) certification.
• Current American Heart Association Advanced Cardiovascular Life Support (AHA ACLS) certification.
• Strong clinical assessment, care coordination, organizational, and communication skills.
• Ability to work independently while collaborating with an interdisciplinary healthcare team.
• Willingness to travel locally across multiple counties within the assigned service territory.
• Must not have applied to or worked for Residential Healthcare Group within the previous 6 months.

Preferred Qualifications

• Previous experience as a Hospice RN Case Manager.
• Experience managing hospice patient caseloads in home-based settings.
• Familiarity with electronic medical records and hospice documentation requirements.
• Experience coordinating interdisciplinary care and supporting nursing staff.
• Ability to provide an Illinois RN license number for credential verification.

Schedule and On-Call Requirements

Regular Work Schedule

• Monday through Friday, generally 8:00 AM–5:00 PM.
• Daily start and end times may vary depending on patient care needs.
• Candidates must be available during the standard daytime work window.

Weekend Coverage

One assigned 24-hour weekend coverage period per month, occurring on Saturday or Sunday.

• 8:00 AM–5:00 PM: Scheduled patient visits and field-based nursing care.
• 5:00 PM–8:00 AM: On-call coverage through the following morning.
• Dedicated after-hours nursing staff are available to provide additional clinical support.

Weekday On-Call Coverage

• Two weekday overnight on-call shifts per month.
• Coverage hours: 5:00 PM–8:00 AM.

Holiday Requirements

• Two assigned holidays annually: one summer holiday and one winter holiday.
• Three floating holidays.

Additional stipends and compensation are provided for qualifying on-call responsibilities.

Annual Base Salary: $80,000

Travel Requirements

Candidates must be comfortable traveling within their assigned geographic service area and managing a schedule of patient visits.
