Home Care LPN Office Nurse

Liberty Health seeks a Home Care LPN Office Nurse to support evidence-based, person-centered services for clients in home and community settings. In this role, you’ll coordinate home care visits, perform telephonic and in-office clinical assessments, triage patient needs, and support interdisciplinary care teams. Responsibilities include medication reconciliation, care plan documentation, communicating with physicians and community partners, and educating patients and families. You’ll help ensure regulatory compliance, maintain accurate electronic health records, and contribute to continuous quality improvement in a collaborative, mission-driven environment.
