Clinical Pharmacist

Responsibilities Determine the suitability of individual patients for home care. Collect and organize all patient‑specific information needed to determine the suitability of patients for home care. Assess patients' suitability for home care in accordance with Option Care policies. Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for the pharmacotherapy of home care patients. When appropriate, make suitable recommendations for modifications in the prescribed medication therapy. Collect and organize patient‑specific information needed to prevent, detect, and resolve medication‑related problems. Complete nutritional assessment of patients and make appropriate recommendation. Modify the pharmacy care plans for home care patients based on evaluation of monitoring data. Provide medication‑use education to Option Care staff and home care patients. Prepare and dispense medications using appropriate techniques and following Option Care policies and procedures. Manage the use, maintenance, and troubleshooting of medication administration equipment. Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. At least one of the following: one year of experience in sterile IV admixture procedures, knowledge of non‑sterile compounding, immunizations and medication therapy management (MTM), completion of an ASHP accredited Pharmacy Residency Program, completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or equivalent experience approved by VP of Pharmacy/SVP of Clinical Services. Access to a reliable means of transportation for home visits. Experience providing customer service to internal and external customers. Basic level skill in Microsoft Word.
