About the Facility – Uintah and Ouray Health Center

Leave the hazards of urban life behind and apply your skills to a wide scope of health care. This ambulatory 40-hour facility is staffed by physicians, physician's assistants, nurse practitioners, and dental, optometry, and psychology providers. They provide comprehensive public health services including general medical, surgical follow-up, pediatric, prenatal, and postpartum care. Dental, behavioral health, and optometry services are also available. Other clinical specialties are provided by visiting consultant and are scheduled periodically. Patients requiring more complex medical care are referred to the hospital or other contract facilities. Facility Address: 6822 E. 1000 S. Ft. Duchesne, UT 84026 Dentist – RESPONSIBILITIES (listed are included but not limited to)

• Will assure preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care.
• Will work places with Allied health professionals to utilize and coordinate the services of other health care professionals in the management of the patient's dental problems.
• Will provide minor dental office surgery such as drainage of a tooth abscess.
• Will provide office based dental care from childhood to adulthood.

Dentist – REQUIREMENTS

• Experience: Minimum of one year of experience as dentist after dental school. One year of Fellowship can count as this experience.
• Education: Must have ADDS or DMD degree from an accredited dental school program, doctorate is required.
• Must have certification of Basic Life Support.
• The Dental must have an active, unrestricted, and free of disciplinary action license as a dental assistant in a State, District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.
