Care Manager: High-Risk Member Health Advocate

Appalachian Community Services is seeking a Care Manager to proactively engage high-risk members and coordinate whole-person health care across providers. The role emphasizes assessment, ISP development, and coordination of services to support members with behavioral health needs.

The ideal candidate has a bachelor’s degree in a health-related field (or RN licensure) and at least 5 years of clinical practice, with demonstrated experience in behavioral health and performance metrics.
