Geriatric Medicine APP

Advanced Practice Provider Location: MaineHealth Geriatric Medicine, Farmington, Maine Schedule: Part-time, 20 hours per week (typically 2 days per week). New graduates are welcomed to apply. Req#: 65508 We are seeking a Physician Associate (PA) or Nurse Practitioner (NP) to provide primary care services in the hospital at MaineHealth Franklin Hospital. The role includes assessing, diagnosing, prescribing, treating, and educating patients. The PA/NP will work closely with physicians and other clinicians in a team approach, primarily in nursing home settings providing care to post-acute and long-term care patients. Responsibilities also include working with geriatricians and other APPs in a shared patient model. The position practices within partnered facilities, including long-term, assisted-living, and skilled rehabilitation care settings in Farmington, Maine. Minimum Qualifications Successful completion of an accredited Nurse Practitioner or Physician Associate/Assistant program. Two years of Nurse Practitioner or Physician Associate/Assistant experience is preferred but not required. Current license issued by the State of Maine or eligibility for licensure.
