Fairview Nursing & Rehabilitation Center is seeking a compassionate, organized, and dedicated Social Services Assistant to join our interdisciplinary team. This is an excellent opportunity for someone who is passionate about enhancing the quality of life for residents in a skilled nursing and long-term care setting.

The Social Services Assistant works closely with the Social Services Director to help residents and their families navigate the admission process, access community resources, and support their emotional, psychosocial, and discharge planning needs.

• Full-Time

• Monday–Friday

• Assist the Social Services Director with resident assessments, care planning, and discharge planning.

• Provide emotional support and advocate for residents and their families.

• Assist with admissions, room changes, transfers, and discharge documentation.

• Coordinate community resources and referrals as needed.

• Maintain accurate and timely documentation in accordance with facility policies and state and federal regulations.

• Participate in interdisciplinary care plan meetings.

• Assist with resident and family concerns and help resolve issues promptly.

• Support resident rights and promote a positive resident experience.

• Collaborate with nursing, therapy, admissions, and other departments to ensure quality resident care.

• Perform other duties as assigned.

• Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.

• Previous experience in a Skilled Nursing Facility, Long-Term Care, Rehabilitation, or healthcare setting preferred.

• Knowledge of discharge planning and community resources is a plus.

• Strong communication, organizational, and interpersonal skills.

• Ability to maintain confidentiality and build positive relationships with residents, families, and staff.
