Northeast Counseling Services (NCS) is seeking a compassionate, motivated, and dedicated individual to join our team as a Blended Case Manager. This position provides services in community settings rather than an office environment, allowing you to directly support individuals where they live, work, and participate in their communities. Flexible schedule is available to help promote work-life balance.

The Blended Case Manager works with individuals experiencing serious mental illness using a strengths-based, recovery-oriented approach. The primary goal of this position is to empower clients to identify their needs, access resources, and achieve greater independence through coordinated support and advocacy.

Must maintain a valid PA Driver's license.

High School Diploma or equivalent is required.

Job Type: Full-time

Pay: $31,200.00 per year to $33,000.00 per year and up based on education and experience

Schedule:

• Monday to Friday

Education:

High School Diploma or GED.

• Minimum of 12 college credit hours in Sociology, Human Services, Psychology, or another social science field is required.
• Two (2) years of experience in Human Services with 1 year of providing direct client contact.
• Bachelor's Degree in Social Work, Psychology, Human Services, Sociology, or a related field preferred

License/Certification:

• Driver's License (Required)

This position requires the successful completion of an FBI Clearance, PA Criminal Clearance and Child Abuse Clearance.
