Registered Nurse (RN) Case Manager - Home Care

Position Overview
The RN Case Manager will be responsible for conducting in-home visits, coordinating and managing patient care, and ensuring compliance with clinical standards and regulatory requirements. This position involves both direct patient care and case management responsibilities, supporting patients across the continuum of care.

Job Details

• Hourly Rate: $65/hr
• Location: Philadelphia- Center City/North Philadelphia or South Philadelphia
• Schedule: Monday through Friday, 8:00 AM - 4:30 PM
• Caseload: 5-6 home visits per day, managing an active caseload of approximately 25 patients

Key Responsibilities

• Perform comprehensive patient assessments during home visits and develop individualized plans of care
• Provide skilled nursing services in accordance with physician orders, agency policies, and state/federal regulations
• Coordinate and manage patient care across disciplines, serving as the primary clinical point of contact
• Accurately complete and maintain OASIS documentation and other required clinical records in a timely manner
• Monitor patient progress, identify changes in condition, and communicate updates to physicians and care team members
• Educate patients and families on disease processes, medications, treatments, and self-care strategies
• Ensure compliance with quality, safety, and regulatory standards
• Advocate for patients to ensure appropriate services and optimal outcomes

Job Requirements

• Active Registered Nurse (RN) license
• Minimum of 1 year of RN experience
• Minimum of 1 year of visiting home care experience
• OASIS experience required
