This full-time position offers the flexibility of remote work, primarily serving seniors within our hospice center. Work Environment\n

This position is primarily remote, allowing you to manage your schedule effectively while meeting the diverse needs of seniors in our community. The standard work hours are Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening and weekend hours to accommodate client needs. As an LCSW, you may also be required to be on call, providing support during urgent situations to ensure continuity of care.

• Conduct in-depth assessments of seniors’ psychosocial needs, covering emotional, mental, and social aspects of their lives.

• Collaborate closely with clients and their families to create personalized care plans that reflect their goals, preferences, and necessary interventions.

• Provide counseling and emotional support to clients, aiding them and their families through life transitions and complex challenges.

• Advocate for clients' rights, ensuring they have access to essential community resources and services that enhance their quality of life.

• Engage in regular visits to monitor clients' well-being, address safety concerns, and assess living conditions, ensuring safe and supportive environments.

• Facilitate communication and mediation among family members to resolve conflicts or misunderstandings related to care decisions.

• Educate clients and families about available community resources, support networks, and engagement opportunities, enabling them to connect with services that promote well-being.

• Maintain meticulous documentation of assessments, care plans, progress notes, and interactions to ensure compliance and high-quality service delivery.

• Stay informed about current best practices, policies, and regulations in social work to ensure ethical compliance in service delivery.

This role specifically focuses on the senior population, particularly those utilizing hospice services. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Valid LCSW state licensure.

• A minimum of two years’ experience in social work, especially in a community center or geriatric context.

Compensation and Benefits\n
