Physician Assistant, In-Home Visits

We are seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations focused on improving patient health outcomes. In this role, you will perform 45-minute visits in patients' homes , including medication and medical history reviews, physical evaluations, and optional diagnostic tests. You will utilize iPads and clinical workflows , without prescribing medication or altering treatment plans. Key responsibilities include building trusted patient relationships, providing personalized health assessments, and collaborating with a national network of clinicians . Qualifications required are active, unrestricted licenses, board certification, and the ability to work in primary locations such as Bellingham and surrounding areas . Multi-state licenses are preferred. Ideal candidates are patient-focused, adaptable to diverse home environments, and comfortable conducting virtual visits as needed .
