Nurse Practitioner

1st Care Management, Inc.

1st Care Management is looking for Nurse Practitioner's (NP) to provide home visits to elderly and home bound patients for the purpose of transitional care. We are flexible and have only PRN positions available at this time.

We understand the importance of primary care and want to assist our patients with gaining access and preventing hospitalization. If this is inline with your goals as a Practitioner, please apply!

We serve Dekalb, Gwinnett, Cobb, North and South Fulton counties.
