MDS Coordinator

Job Summary:

Northview Health and Living is seeking a dedicated and compassionate MDS Coordinator to join our team in Anderson, IN. As an MDS Coordinator, you will play a crucial role in ensuring the accuracy and timeliness of Minimum Data Set (MDS) assessments, care plans, and quality measures for our residents. Responsibilities:

• Coordinate and conduct MDS assessments, care plans, and quality measures for residents in our skilled nursing facility

• Ensure timely and accurate completion of MDS assessments and care plans, meeting established deadlines and quality standards

• Collaborate with interdisciplinary teams, including physicians, nurses, and therapists, to develop and implement individualized care plans

• Maintain accurate and detailed records of resident assessments, care plans, and quality measures

• Analyze data and identify trends to improve care processes and outcomes

• Provide education and training to staff on MDS and care planning processes

• Participate in quality improvement initiatives and quality management activities

• Ensure compliance with federal, state, and accrediting agency regulations and standards

Requirements:

• Minimum Requirements: Minimum 2 yrs of experience.

• Must be an LPN

• Strong organizational and communication skills

• Ability to work effectively in a fast-paced environment

• Strong attention to detail and accuracy

• Ability to work collaboratively with interdisciplinary teams
