We are currently seeking skilled and compassionate Physical Therapists to join our team at our partner's skilled nursing facility. Responsibilities Provide physical therapy services to residents in a skilled nursing facility, focusing on restoring mobility and improving functional independence. Develop and implement individualized treatment plans in collaboration with the care team. Document progress and communicate with residents, families, and staff. Participate in discharge planning and care planning meetings. Promote resident-centered care and uphold professional standards and ethics. Qualifications Physical Therapist license in good standing (state-specific requirements). Strong communication and teamwork skills. Commitment to quality patient care and continuous professional development. Experience in rehabilitation or geriatric settings preferred.
