Per Diem Weekend Nurse

The position includes weekend availability. Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest. The ideal candidate is well‑organized and enthusiastic about working in a fast‑paced environment. Job Description The Per Diem Weekend Nurse is an integral part of an interdisciplinary hospice team responsible for caring for patients at the end of life. The role includes making home and hospital visits, providing information sessions, evaluating new patients, facilitating admissions to hospice services, and coordinating care planning. The nurse employs triage and assessment skills to titrate medications and manage symptoms appropriately and provides education to patients and families about terminal disease and end‑of‑life process. The nurse also communicates with an interdisciplinary hospice team about changes in patients’ needs and outcomes of interventions.
