- Serve as a member of the ID Team which designs individualized plans for each client to ensure effective methods of behavior modification and treatment.
- Conduct group sessions to clients specified in the Individual Support Plan (ISP), to include but not limited to: crisis management and psychoeducational groups.
- Collects, computes and analyzes behavior program data.
- Provides staff in-service training to direct and/or support staff, to ensure proper operation of behavioral programs.
- Provides monthly progress notes based on the plans, observations and attendance.
- Utilization of crisis prevention and intervention utilizing non-violent crisis intervention techniques and SDDC policies and procedures.
- Member of Peer Review Committee, review and approve behavior support plans containing limitations and/or highly restrictive procedures.
- Represent Behavioral Health Team at, but not limited to: Human Rights Committee, Psych Consults, Medication Reviews, Supplemental Team Meetings, Annual Individual Support Plan meeting.
- Assist Behavioral Health Team in Functional Behavioral Assessments as directed by Psychologist or Behavior Analyst.
- Author evaluations utilizing person centered practices and integrated services and support domains for inclusion in the comprehensive functional assessment.
- Authoring behavior support plans without limitations. Co-author behavior support plans with limitations and/or highly restrictive procedures.
- Review administration of emergency interventions, safety frame record, safety precaution forms and incident report forms. Retain documents per retention manual.
- Participate in transition and discharge planning.
- Scoring various standardized tests that measure emotional, cognitive and behavioral functioning.
