Case ManagerThe role of a Case Manager is essential in providing support and coordination for patients in an acute freestanding rehabilitation setting.Shift and HoursShift: 8:00 AM - 4:30 PM, Monday to FridayWeekly Guaranteed Hours: 40Weekend Hours: 0Call Requirements: 0QualificationsCurrent licensure as a Registered Nurse, Social Worker, or other healthcare professional such as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist, along with relevant case management experience.2-3 years of experience preferred.Certifications in case management are preferred, such as:Commission for Case Manager Certification (CCM)Association of Rehabilitation Nurses (ARN) certificationAmerican Case Management Association (ACM)Board Certification in Case Management (RN-BC)Effective oral and written communication skills in English; proficiency in additional languages is a plus.Basic computer skills in Excel, Word, Outlook, etc. are required.Key ResponsibilitiesCollaborate with nursing, therapy teams, and medical professionals to support patient care.Engage in family conferences and utilize reviews to optimize patient outcomes.Advocate for patients while balancing resource management and healthcare realities.Participate in eligibility assessments for acute rehabilitation criteria.Personality FitStrong communicator able to work effectively in a diverse team.Demonstrates a balance between patient advocacy and resource stewardship.Capable of handling challenging discussions professionally with families and patients regarding insurance and healthcare realities.EnvironmentNumber of Beds: 60Patient Ratios: 1:15 (±)Electronic Health Record: UDS PRO, MediTech, and PinpointFirst Day InstructionsScrub Color: Business Casual / Hospital ProfessionalTime to Report: 8:00 AMContact Person Upon Arrival: Shanna MeadorParking: Available in designated patient parking areaClient Details City Tacoma State WA
