What You’ll Do

Provide skilled nursing care in patients’ homes under the direction of the Registered Nurse and physician

Perform ordered treatments, medication management, wound care, and patient education within the LPN scope of practice

Observe, document, and report changes in patient condition to the supervising RN

Collaborate with physicians, nurses, therapists, and other members of the care team

Complete accurate and timely documentation while maintaining Medicare and agency standards

Qualifications

Current Florida Licensed Practical Nurse (LPN) license

Valid driver’s license and reliable transportation

Excellent clinical, communication, and organizational skills

Home health experience preferred, but not required

Experience with WellSky or another home health EMR is a plus
