We positively impact thousands of lives each year with the "Ohana Experience", an organizational culture based on excellence and genuine care.Hawaii's largest, locally owned post-acute healthcare company.Our vision is to transform how healthcare is provided throughout Hawaii.The Position:We are looking for a passionate Physical Therapist (PT) to develop and implement a progressive rehabilitation program for patients in their homes in accordance with the principles and practices of physical therapy and the client's needs. Serves as case manager and supervises home health aides and physical therapist assistants when appropriate.Evaluates assigned patients. Admits those who meet agency and regulatory criteria.Plans, coordinates, and implements the appropriate treatment according to patient's needs. Performs ongoing reassessments. Discharges patients when goals are met, or patient does not meet home health requirements.Develop Home Health Aide (HHA) and Physical Therapy Assistant (PTA) care plans. Delegates duties to HHA's and PTAs (Physical Therapy Assistant) based on agency and regulatory policies. Supervises and provides input to HHA's and PTAs per agency policy.Assumes responsibility for case management of assigned patients. Consults with appropriate community and internal resources and makes appropriate interdisciplinary referrals.Performs patient care duties and/or treatments based on specific needs, including medical condition, age, and/or developmental status.Requirements:Master's Degree or Doctorate Degree in Physical TherapyHave successfully completed and graduated from an accredited occupational therapy professional program recognized by NBCOT and have completed all fieldwork requirementsCurrent medical/health screening including 2 step TB clearance.Valid State of Hawaii license to practice as an Physical Therapist.Current Basic Life Support CPR (cardiopulmonary resuscitation) certification through the American Heart AssociationTwo years experience as a physical therapist, preferably in home health or other healthcare practice setting.The Benefits:$2,000 Retention Bonus!Relocation Assistance may be availablePaid Time Off (PTO)Tuition Reimbursement & Scholarship OpportunitiesMedical/Dental/Vision/401(k) matchExcellent growth and advancement opportunitiesMission Driven Ohana & Dedicated Team CultureCompetitive pay and benefitsOhana Pacific Health is an Equal Opportunity Employer.
