POSITION SUMMARY: The primary purpose of your job position is to perform restorative nursing procedures that maximize the resident’s existing abilities, emphasize independence instead of dependence, and minimize the negative effects of disability with an attitude of realistic optimism under the supervision of a restorative nurse. You will also assist in modifying the treatment regimen to meet the physical and psychosocial needs of the resident, in accordance with established medical practices and the requirements of this state and the policies and goals of this facility.
ESSENTIAL DUTIES AND RESPONSIBILITIES : Every effort has been made to identify the essential functions of this position. Plan, develop, organize, implement, evaluate, and direct restorative care services, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
Meet with administration, medical and nursing staff, therapy department, as well as other related departments in planning and restorative care.
Interpret restorative care policies and procedures to personnel, residents, family members, etc., as necessary.
Participate in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary.
Assist the Quality Assessment and Assurance Coordinator in developing, implementing and maintaining an ongoing quality assurance program for restorative care services.
Assist the Quality Assessment and Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
Assume the authority, responsibility, and accountability of directing restorative care services.
· Provide written and/or oral reports of the restorative care programs and activities as required.
· Evaluate and implement recommendations from established committees as they may pertain to restorative care.
· Review request for restorative care and physicians’ orders.
· Interview resident to determine type of treatment and schedule therapy.
· Select rehabilitative and therapeutic activities and exercises based on medical and social history of residents.
· Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly reviews, as they relate to restorative care.
· Brief residents of procedures involved in restorative care and prepare necessary equipment.
· Demonstrate to residents and staff personnel, as necessary, the procedures involve in the treatment process.
· Perform restorative care treatments in resident’s room as necessary.
· Prepare resident for treatment by dress/position and administer restorative care in accordance with established policies and procedures.
· Encourage residents to perform or participate in ADL functions.
· Encourage residents to perform range of motion exercises.
· Instruct and supervise resident during therapy.
· Assist residents in ambulatory training as necessary.
· Recommend modifications and changes in the resident’s restorative care as indicated.
· Ensure that all restorative care notes are informative and descriptive of the treatment provided and of the resident’s response to the care.
· Assist in developing safety standards for restorative care.
· Assist in developing restorative care plans for individual residents in coordination and conjunction with the resident assessment (MDS).
· Review the restorative care requirements of each resident admitted to the facility and assist the attending physician in planning for the resident’s care and/or treatment.
· Ensure that care plans indicate restorative care when such therapy is ordered.
· Ensure that therapy personnel knock before entering the resident’s room.
· Ensure that all restorative care personnel are knowledgeable of the residents’ responsibilities and rights including the right to refuse treatment.
· Ensure that therapy personnel honor the resident’s refusal of treatment. Report such requests to the Director of Nursing Services.
· Abide by the resident’s participation in treatment decision.
· Inform the resident of consequences of not participating in prescribed therapy and document such action in the resident’s clinical record.
Performs other duties as may be assigned.
QUALIFICATIONS: Education and/or Experience:
· Graduate of an accredited Licensed Vocational or Practical Nurse program, or with a nursing degree from an accredited college or university.
Certificates and Licenses:
· Must possess an active license to practice as a Registered Nurse, Licensed Vocational Nurse or a Licensed Practical Nurse valid in this state.
Other Specific Requirements
· Must provide evidence of being free of tuberculosis infection upon hire and as set forth by the policies of the facility.
· Must be able to read, write, speak and understand the English language.
· Must possess the ability to make independent decision when circumstances warrant such action.
· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
· Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
· Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
· Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
WORK ENVIRONMENT:
Works throughout the nursing service area (i.e. nurse’s stations, resident rooms, etc.).
Moves intermittently during working hours.
Is subject to frequent interruptions.
Interacts with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
Communicates with the medical staff, nursing personnel, and other department supervisors.
Works beyond normal working hours and on weekends and holidays when necessary. May be on call 24 hours per day, 7 days per week.
Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
Is subject to injury from falls, burns from equipment, odors, etc., as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
May be subject to the handling of and exposure to hazardous chemicals.
PHYSICAL AND SENSORY REQUIREMENTS (with or without the aid of mechanical devices):
· Must be able to move intermittently throughout the work day.
· Must be able to cope with the mental emotional and stress of the position.
Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
Must function independently, have flexibility, personal integrity, and the ability to work effectively.
Must meet the general health requirements set forth by the policies of this facility which may include a medical and physical examination.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
Must be able to lift up to 25 pounds. Requires infrequent lifting of more weight up to 50 pounds. Occasional pushing and pulling of carts.
· May be necessary to assist in the evacuation of residents during emergency situations.
