We are looking for a qualified Hospice and Home Health RN Case Manager that shares our values and wants to assist in our vision to provide the excellent care that the community expects from us. We pride ourselves on very low employee turnover. We are looking for a qualified RN Case Manager who wants a long term position with a great company. As a Hospice and Home Health RN Case Manager your duties will include but not be limited to:Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.Create patient centered goals with patients, family members and providersAssess, supervise and direct patient careDevelop plans of treatmentEnsure chart compliance with state regulationsCoordinate patient care with physicians and other providersHospice and Home Health are both challenging and rewarding! Other requirements include:Licensed as a Registered Nurse by the State of UtahCurrent Utah driver's license and automobile insuranceCPR certificationA minimum of 1 year nursing experience is requiredHospice and home health experience is strongly preferred but not requiredRequired education: Associate degreeRequired experience: Nursing: 1 yearRequired license or certification: Registered Nurse (RN) If you have a passion for providing high quality patient care, have good documentation skills, and enjoy caring for patients in a home setting, then this is the job for you and we look forward to hearing from you.
