To apply for the CT and XR Technologist position, you must have the following credentials and documentation. License or certificate verification is required for submission. A driver's license or state ID and two professional references are also needed for submission. The onboarding process requires various health history records, including MMR, Hepatitis B, a fit test, a health statement, 2 Step PPD or IGRA, Varicella, and Tdap. Additionally, a criminal background check, Ascension skills checklist, 12-panel drug screen, education verification, confirmation of assignment, and employment verification must be completed. Optional submissions include an influenza vaccine, positive PPD with a negative chest X-ray, an annual TB questionnaire, and a hiring manager evaluation.
