Job Responsibilities: Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards. Utilizing the state tracking system HHH (EMR like tool used by DPH) after receiving comprehensive training. Completing assessments for clients within 24 hours of their family's arrival into the system. Providing on-site medical support as needed by CSR ((Clinical and Safety Risk Assessment) families. Assisting CSR families with medical referrals, appointment setup, and access to required medical services. Educate patients and their families on health maintenance and disease prevention. Ensure accurate documentation and compliance with all regulatory guidelines. Requirements: Active RN license in the state of practice. Previous experience on patient admission and doing assessment. Exceptional interpersonal and communication skills. Committed to delivering high‑quality, patient‑centered care.
