Licensed Practical Nurse

Your future role at a glance Location: Sapulpa, OK Facility: Ascension St. John's Medical Center Department: Med/Surg Swing Schedule: Full-time, Nights, 7pm-7am, with every other weekend How you'll make an impact in this role Deliver Comprehensive Bedside Care: Administer medications, provide wound care, and perform routine clinical procedures for acute med/surg patients and skilled rehabilitation residents. Monitor and Document Patient Status: Conduct focused physical assessments, track vital signs, and maintain precise electronic health records (EHR) to capture changes in patient condition. Support Care Transitions: Collaborate with RNs and case managers to execute tailored care plans, assisting patients as they transition from acute hospital care to post-acute recovery or discharge. Educate Patients and Families: Provide clear instructions regarding post-discharge care, medication schedules, and rehabilitation goals to promote optimal recovery and independence. What minimum qualifications you'll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Practical/Vocational Nurse credentialed from the Oklahoma Board of Nursing required. Education: High School diploma equivalency or 1 year of applicable cumulative job specific experience required. Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
