Coordinator Discharge Planning- 40/hrs, Day Shifts (Temple Hospital)

DC Planning Coordinator
The coordinator of DC planning is responsible for designing and coordinating comprehensive discharge plans in accordance with regulatory and patient satisfaction standards. Collaborates with nurse case managers and social workers regarding overall discharge planning needs.
Education:
- Graduate of a licensed practical nurse program or
- Graduate of an accredited Physical Therapy Assistant Program or
- Bachelors of Social Work Degree Required
Experience:
- 1 year experience with referral coordination Preferred
- 2 years experience in a Hospital Clinical setting Preferred
Licenses:
- PA Physical Therapist Asst Lic Required or
- PA Practical Nurse License Required
