As a non-profit, community-led agency, we are able to focus on providing quality care to patients and families when they need it most, rather than being driven by profit or financial demand.This full-time position offers competitive pay, generous benefits and a supportive work environment.We are seeking a Hospice Nurse Case Manager to be responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician's plan of treatment and orders.Schedule:Monday - Friday; 8:30am - 5:00pmLocation:Contra Costa County & some areas of Alameda CountyHope Hospice follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to):Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process. Maintain a caseload and productivity per agency's protocol.Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.Maintain regular communication with Director of Patient Services/designee to review caseload.Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.Education/Experience:One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing.Experience in acute care/ home health and/or hospice setting preferred.Certificates, Licenses, Registrations:Current license as a registered professional nurse in the State of California.Public Health Nursing certification highly desirable.HPNA certification preferred.Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car.Pay Range: $58.90 - $73.65 hourlyHope Hospice is proud to serve our community as an equal employment opportunity employer.
