Discharge Planner Social Worker

Discharge Planner / Social Worker Fairview Skilled Rehabilitation & Nursing Center is seeking a compassionate and organized Discharge Planner / Social Worker to coordinate safe and timely discharges for our residents. This position works closely with residents, families, physicians, nursing, therapy, and community providers to ensure a smooth transition from our facility to the next level of care. Responsibilities Coordinate discharge planning from admission through discharge. Complete psychosocial assessments and provide emotional support to residents and families. Arrange home care, medical equipment, transportation, and community services. Communicate with hospitals, physicians, insurance companies, and outside providers. Maintain accurate documentation and ensure compliance with state and federal regulations. Participate in care plan meetings and collaborate with the interdisciplinary team. Qualifications MSW preferred; LMSW/LCSW preferred. Experience in skilled nursing, rehabilitation, hospital, or case management preferred. Strong communication, organizational, and teamwork skills. Knowledge of Medicare, Medicaid, and discharge planning is a plus.
