Physical Therapist - Travel Contract

Minimum Requirements
Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active California Physical Therapist license required to start the assignment.
BLS certification from AHA or ARC may be required.
Minimum 1 year of home health experience with a Medicare-certified facility (client requirement).
Experience using HCHB (Homecare Homebase) EMR required (client requirement).
Productivity expectation: approximately 30 points per week (client requirement).
Ability to meet schedule expectations of a full-time, 40 hours per week assignment (client requirement).
Preferred: prior experience with Medicare documentation and home health outcomes tracking.

Facility Type: Home Health.
Shift Name: Days.
Shift Details: Days. 8-hour shifts, Monday through Friday. Schedule example listed as 8:00 AM–5:00 PM.
Guaranteed hours per week: 40.
EMR: HCHB required.
Productivity requirement: 30 points per week.
Assignment length: 13 weeks.
