Prelicensed Social Worker

Role Overview: Provides psychosocial services to patients undergoing dialysis, supporting their emotional, psychological, and behavioral well-being. Collaborates with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients with Chronic Kidney Disease (CKD). Assists in care planning, offering counseling, education, and referrals to community resources to address barriers and enhance treatment adherence. Supports transplant education and referral processes, and participates in quality improvement activities related to patient satisfaction and outcomes. Key Responsibilities Patient Assessment & Care Planning: Evaluates psychosocial status, identifies needs, and collaborates on interventions to meet treatment goals. Support & Counseling: Provides supportive counseling and educational services to patients and families, addressing psychosocial issues and treatment adherence. Patient Education: Educates patients on kidney disease, treatment options, transplant process, and patient rights, ensuring understanding of available support services. Admission & Continuity: Assists with patient intake, addresses immediate concerns, and supports smooth care transitions. Insurance & Financial Assistance: Acts as liaison for insurance issues, provides information on payment options, and refers patients to billing support. Team & Staff Support: Participates in team meetings, supports staff training, and contributes to process improvements. Qualifications & Skills Master’s in Social Work with required state licensure or provisional license. 0-2 years of related experience. Strong assessment, counseling, and educational skills. Ability to collaborate in a multidisciplinary team and support patients' psychosocial needs.
