Street Medicine Peer Navigator: Housing & Health Advocate
Healthcare in Action
Healthcare in Action is seeking a dedicated Peer Navigator to support members experiencing homelessness in the Los Angeles area. The role focuses on education, peer support, housing resources, and care coordination within ECM and CalAIM Community Supports. The position is full-time, Monday–Friday, with field duties and potential 50% travel.
