Public Health Care Manager - CMHRP

Public Health Care Manager - CMHRP (Care Management for High-Risk Pregnancies) Hiring Range: $57,346 – $75,266.63. This exempt (salaried) position is funded through December 31, 2026 with renewal options based on available funding. The role is on-site, five days a week, 8:00 AM to 5:00 PM. Responsibilities Provide professional-level care management services for various populations. Coordinate care across the healthcare continuum, evaluating, counseling, and referring clients as needed. Link and arrange services and advocate to implement person‑centered care plans. Communicate with doctors and specialists, ensuring timely delivery of services and treatments. Interview clients (individual, family, or group), gather social, medical, and family information, and develop client‑centered care plans. Determine appropriate referrals, client eligibility for sponsored programs, and assist with completion of required paperwork. Act as liaison with internal and external staff, ensuring completion of referrals and service provision. Maintain case history records, prepare reports and documents demonstrating program progress. Provide education and support to clients on topics such as child‑rearing, job placement, debt counseling, legal aid, housing, medical treatment, and financial assistance. Serve on program‑specific policymaking committees and assist community partners, advocating for solutions to client problems. Support MCHD response activities, including public health emergencies and student rotations. Minimum Qualifications Minimum of one year of public health, healthcare, and/or regulatory experience. For social workers: MSW from an accredited school or BSW with one year of social work experience. For nurses: BSN from an accredited nursing program or ADN with relevant experience. Preferred Qualifications BSN with one year of nursing care management experience. ADN with three years of nursing care management experience. Licenses and Certifications Current certification in Basic Life Support for Healthcare Providers (CPR & AED) valid for at least 90 days after hire. Valid North Carolina or South Carolina driver’s license and County driving privileges. Current license to practice as a Registered Nurse (RN) in NC issued by the NC Board of Nursing or another Nurse Licensure Compact State. Knowledge, Skills, and Abilities Knowledge of social services procedures, practices, and federal/state/local laws. Knowledge of care management methods, local public and private service agencies. Knowledge of medical and legal terminology and principles of public health. Ability to prioritize work, manage multiple tasks, maintain confidential records, and apply judgment and discretion. Strong communication, conflict management, decision making, and partnership building skills. Proficient in Microsoft Office Suite and other relevant computer applications.
