Nurse Practitioner or Physician Assistant

Matrix Medical Network

Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant to perform comprehensive in-home health assessments, chronic disease management, and preventive care for diverse patient populations. In this role, you will leverage evidence-based practice, electronic health records, and strong clinical judgment to evaluate risks, close care gaps, and coordinate with primary care and health plans.
