The role focuses on Medicaid and Medicare reimbursement, coordination and completion of clinical assessments, supporting documentation, care planning, and transmitting MDS 3.0, in compliance with federal and state regulations. The coordinator participates in and directs the delivery of patient care in accordance with facility clinical systems, regulatory requirements, and physician orders, ensuring the highest quality services in a customer-service-friendly environment. The role contributes to a positive patient experience by providing quality compassionate healthcare and supports the facility’s mission to deliver superior clinical care, rehabilitation, wellness, and supportive services that meet patient and resident needs and expectations.
