Physical Therapist – Home Health

Contract | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across the West Portland, OR area and surrounding regions
• Conduct patient evaluations and deliver individualized treatment plans with a 1:1 patient ratio, visiting up to 6 patients per day
• Complete clinical documentation using EPIC Remote Client for Home Health and Rover mobile application, including OASIS assessments
• Manage daily patient visit schedules independently, utilizing personal transportation to travel to patient homes within an approximate 60-mile service radius
• Meet standard productivity expectations of 30 productivity points per day

Required Qualifications

• Active Oregon (OR) and Washington (WA) Physical Therapist licensure — both licenses required
• Minimum 1 year of skilled home health physical therapy experience
• OASIS documentation experience required
• Current Basic Cardiac Life Support (BCLS) certification
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required

Preferred Experience / Skills

• Familiarity with EPIC Remote Client and Rover mobile application for home health charting
• Experience meeting productivity point expectations in a home health environment
• Comfort managing an independent daily schedule with variable drive times
