Bilingual Targeted Case Manager

HEALTH CONNECT AMERICA, INC is seeking a Bilingual Targeted Case Manager to support children, adults, and families with behavioral health needs. In this role, you will assess client needs, develop service plans, coordinate referrals, and advocate for clients across community, in‑home, and office settings. You will collaborate with therapists, clinicians, and community partners to ensure comprehensive, culturally responsive care. Responsibilities include monitoring progress, maintaining accurate documentation, and linking clients to social, educational, and healthcare resources while supporting recovery and stability in their local communities.
